Other Forms for Planters Church Charter/Affiliation Application – If you would like to charter/affiliate your church with Open Bible Churches, please contact Ken Groen at the Central Region office by calling 1-877-482-6491 or emailing cenregobc@aol.com Church Planter Monthly Report – Church Planters receiving financial support from OBC are expected to fill this out and submit it monthly. Grant Disbursement Proposal Worksheet (for OBC-approved planters only– Use this spreadsheet to project how grant money received from the region would be used. Budget/ Cash Flow Worksheet -- Use this spreadsheet to project your income and expenses for the next year, or modify and use it to keep track of your actual income and expenses. Pioneer Pastor Tithe Rebate Form (for OBC Board approved planters only) – Pioneer Pastors may apply for a rebate of their ministerial tithe during the beginning of their church plant. This rebate would go to the church being pioneered. Pioneer Church 5% Rebate Form (for OBC Board approved planters only) – New church plants can apply for a rebate of their 5% plan monies during the beginning of the church plant. OBC Service Foundation Funding Application for Church Planters (for OBC Board approved planters only) – This foundation has monies available for small, one-time grants to church planters who apply. DISC Profile Questionnaire (coming soon) Form SS-4, EIN (Employee Identification Number) Application Form -- When a planter incorporates his/her church, he/she will need to apply for an EIN. Articles of Incorporation (sample) -- This sample will help a church plant with their own articles.
